Grace Reeves is a New York State Licensed Mental Health Counselor with Diagnostic Privilege and the Manager of Client Care Coordination at Dr. Fallon’s Practice. In her current role, Grace oversees client care coordination and works closely with both clients and clinicians to support continuity of care, effective communication, and a positive experience throughout treatment.

Grace earned her Master’s degree in Clinical Mental Health Counseling and Bachelor’s degree in Psychology with a minor in Biology from Marist College. She continues to pursue professional development and continuing education opportunities to strengthen her knowledge and skills and provide the best possible care and guidance to clients and clinicians.

Grace brings a thoughtful, organized, and collaborative approach to her work. Her experience in both direct clinical care and team leadership allows her to approach her work from a broad perspective, balancing the individual needs of clients with the coordination and collaboration required to provide effective, high-quality care.

Grace is passionate about utilizing her clinical background and skills to inform her approach to client care coordination and leadership. She values clear communication, organization, accountability, and building strong working relationships that support both clients and the clinical team.
